Dani Pedrosa claimed pole position for the Italian Grand Prix at Mugello, but the qualifying session was overshadowed by a heavy crash which left reigning champion Valentino Rossi with a badly broken leg.
Fiat Yamaha rider Rossi suffered a compound fracture of his right shinbone when he fell at the 13th turn of the Tuscan circuit with 15 minutes remaining of the morning's final practice session.
The Italian underwent an operation at a hospital in Florence and initial indications from MotoGP's medical team suggest he could be sidelined for up to three months.
Rossi was chasing a 10th career win at Mugello and was favourite to claim pole position after setting the pace in the opening practice session, but his absence from the afternoon's qualifying paved the way for Pedrosa to grab top spot.
The Repsol Honda rider claimed his second pole of the season - and 15th of his career - with a time of one minute 48.819 seconds.
That was good enough to edge Rossi's team-mate Jorge Lorenzo into second place by just 0.177secs, although the in-form Spaniard will fancy his chances of claiming a third successive victory.
Lorenzo leads the way in the riders' standings by nine points from the stricken Rossi, and the title is arguably already his to lose with his nearest rival set to miss up to eight races following his fall.
"I want to say how sad I am for Valentino," said Lorenzo.
"I know how bad it is to be injured, especially at your home race, and it looks very painful for him.
"I hope he recovers quickly because the championship will not be the same without him."
Lorenzo added: "I have a good race pace, the Bridgestone tyres feel good and I think I can make a fight tomorrow.
"We have to stay calm, focused and concentrated and hopefully it will be a good race for us here in Italy, at the home track for many of my team."
Third place went to Casey Stoner, the Ducati Marlboro rider 0.613s behind Pedrosa, while the Australian's team-mate Nicky Hayden heads the second row from Colin Edwards (Monster Yamaha Tech 3) and LCR Honda's Randy de Puniet.
Ben Spies (Monster Yamaha Tech 3) will line up seventh, while Andrea Dovizioso (Repsol Honda), Aleix Espargaro (Pramac Racing) and Hiroshi Aoyama (Interwetten Honda) completed the top 10.
